Our Home Care Package Assessments help older Australians identify the supports, equipment and strategies that may help them remain safe and independent in their own homes.
We assess areas including daily living activities, mobility, home safety, falls risk, cognitive functioning and environmental barriers.
Following the assessment, recommendations may include assistive technology, home modifications, support services and strategies designed to improve safety and independence.
We work with clients, family members, care managers and aged care providers to ensure recommendations align with individual goals and available funding.
